Sinusitis & Acid Reflux Problems

Acid reflux also known as Gastroesophageal reflux disease (GERD) is a condition that is characterized by chronic irritation of the lining of the esophagus by stomach acid. Chronic heartburn, regurgitation and nausea are signature symptoms of standard cases of acid reflux or GERD. However, some cases are difficult to diagnose and do not present with the standard symptoms. Common symptoms of acid reflux include: pain when swallowing, bad breath, bad taste in the mouth, burping, hoarseness and sore throat. Symptoms must be present at least twice a week on a regular basis. Acid reflux is a chronic condition that occurs when stomach acid passes up through the esophagus often causing pain and / or tissue damage. Some cases of acid reflux also develop further complications such as esophagitis. Sinusitis may develop from acid reflux as well, as acid can potentially enter the nasal airways due to the close proximity of the nasal passages and the mouth which are directly connected. Recent research indicates that sinusitis and acid reflux as well as some other conditions such as chronic ear infections are due to acid reflux, under an atypical diagnositic criteria. There is a subgroup of acid reflux or GERD that is referred to as laryngopharyngeal reflux (LPR) or extraesophageal reflux (EERD) which does not produce heartburn but does impact the respiratory system and the larynx. The signs and symptoms of LPR include cough, postnasal drip, difficulty swallowing, chronic throat clearing, hoarseness, and asthma. LPR is also referred to as the “silent reflux” and is characterized by stomach acid that pools in the throat and larynx. If left untreated LPR can produce scar tissue in the throat and voice box. Sinusitis is inflammation caused by a blockage in the sinus cavity that develops during the course of a cold or an allergy. The result of having a blockage of bacteria, fungi or virus is excess mucus production in the sinus cavities and inflammation due to the pathogens attacking the sinus cavity openings. When the sinus cavity openings are attacked this results in more inflammation and infection. Sinusitis takes place during the course of other infections in the sinus cavities resulting in increased inflammation of the existing inflammation. Sinusitis develops when bacteria get into the sinus cavities and attack the swollen cavity lining, causing more inflammation. During a sinusitis infection, the cilia which are small hairs in the nasal passageways cannot perform their function of removing the debris that is blocked inside the sinus cavity. There are a myriad of symptoms that take place with a sinusitis infection. Common symptoms of a sinusitis infection include: cough, upper respiratory congestion, postnasal drip, nasal discharge, fever, facial pain and facial pressure. Acid reflux develops when there are abnormal relaxation changes in the lower esophageal sphincter that typically act as a barrier between the top of the stomach and the esophagus. As a result of the esophageal sphincter not holding the stomach acid at bay, the acid can back up into the pharynx (throat) or even into the back of the nasal airway. This can cause inflammation in areas that are not protected against gastric acid exposure. The inflammation that develops can help induce a sinusitis infection. There is a co-morbidity with acid reflux and sinusitis, stemming from the inflammation that develop from gastric acid in the throat. The connection between sinusitis and acid reflux usually stem from an atypical diagnosis of GERD or LPR. The stomach acid that pools in the back of the throat can cause long term damage such as increasing risk of cancer as well as irritating pre-existing conditions of asthma, bronchitis, and emphysema if left untreated. · There is not a single medication that can treat both sinusitis and acid reflux. Physicians typically recommend some form of antacid to neutralize and prevent heartburn and reflux such as Pepcid or Tagamet. When over the counter measures fail to treat GERD and LPR, a prescription medication that can reduce the stomach’s production of acid, like Reglan or Protonix will be prescribed. Cases that are not responsive to medication and diet change can result in repairing the esophageal sphincter. Relying on the sinus cavities to provide the medication through the limited supply of blood vessels is problematic as the blood vessels cannot deliver an effective dose of medication to the sinusitis infection. Recurrent sinusitis infections are common with the use of oral antibiotics due to the blood vessels inability to provide an effective dose to the site of infection. The sinus cavities scant supply of blood vessels which are swollen and infected also providing all the treatment to the sinusitis infection results in an ineffective method for treating this area of the body.
